PRP vs Mesotherapy vs Exosomes: Which One Is Right for Your Hair?

pvp vs mesotherapy vs exosomes

PRP, mesotherapy, and exosomes are sometimes used almost interchangeably, sometimes pitched against each other, and often without anyone actually explaining what makes them different.

All three work with your scalp’s biology rather than against it, and all three have a place in hair restoration. But what about the question: “which one is best?” The answer depends on your hair loss pattern, how much certainty you want from the evidence, and what kind of treatment experience you’re looking for.

The short answer: PRP has the longest track record and the strongest evidence base, which makes it a dependable, well-understood starting point for most people. Mesotherapy gives your doctor more room to customize the treatment to your scalp’s specific needs. Exosomes are the newest and most scientifically exciting of the three and the most promising, yet with limited research to date. None of these is the “wrong” answer.

Now, let’s dive deeper.

What each one actually is

It helps to think of these as three different delivery methods, each carrying a different kind of biological signal to your scalp.

PRP (Platelet-Rich Plasma) uses your own blood. A small sample is drawn, spun in a centrifuge to concentrate the platelets, and injected into the scalp. Platelets carry growth factors which are the proteins your body uses for repair. They are the working theory behind why PRP helps revive weakened follicles.

Mesotherapy is a broader category. Rather than using your blood, it delivers a custom-formulated cocktail of vitamins, peptides, amino acids, and other actives, directly into the scalp. Because the formula isn’t fixed, your doctor can tailor it depending on what your scalp and hair need.

Exosome therapy is the newest of the three. Exosomes are tiny vesicles (biological “messengers”) that carry signals between cells. The treatment delivers these signals to your scalp to encourage healthier cell communication and tissue repair. It’s a genuinely innovative approach, building on some of the most interesting regenerative science happening right now.

“I like to explain it this way: PRP is your own biology, concentrated and reapplied. Mesotherapy is a custom-built nutrient delivery. Exosomes are pure cellular messaging. They’re not competing technologies so much as three different tools and which one I reach for depends entirely on what’s happening with that particular patient’s scalp.” — Dr. Ludmila Yaghorova, Medical Director, 7 Derma Center Dubai

What does the evidence actually say?

We think being upfront here matters more than picking a “winner”, so here’s the real picture.

PRP currently has the longest clinical track record of the three, and the most research behind it. Multiple studies and reviews show it can improve hair density and reduce shedding in pattern hair loss, particularly when follicles are still active. If you want the option with the deepest, most consistent body of evidence behind it, PRP is that option.

Mesotherapy has supportive research too, but because the injected formula varies from clinic to clinic, it’s harder to compare results across studies in the same standardized way. Its goal is to be a personalized treatment. What it means practically is that mesotherapy’s effectiveness often comes down to the skill and formulation choices of your specific provider.

Exosomes are where the science is moving fastest and where the excitement is well-deserved. Early research consistently describes exosome therapy as promising for hair and skin regeneration. What’s still developing is the long-term data and product standardization across the industry, simply because it’s a newer field. We continue to offer exosome therapy at 7 Derma Center because we’ve seen encouraging results in our own patients, and because we believe in following where regenerative medicine is heading — while being transparent that it’s the newest member of this family, with research still catching up.

“I tell patients that exosomes are one of the most exciting things happening in regenerative aesthetics right now. We wouldn’t offer it if we didn’t believe in it. At the same time, I think it’s only fair to say that PRP has simply been around longer and been studied more. That’s not a knock against exosomes, it’s just where the science currently stands.” — Dr. Ludmila Yaghorova

What about cost over time?

This is a question we get often, because the per-session price doesn’t always tell the full story. What matters more is how often you’ll need to come back.

What about cost over time?

Typical maintenance pattern Long-term cost tendency
PRP One top-up every 6–12 months after your initial course Lowest to moderate
Mesotherapy Often repeated more frequently — sometimes every few months Moderate
Exosomes Fewer sessions typically needed, though each is priced at a premium Higher

If long-term budget is your main consideration, PRP tends to be the most economical path simply because it needs less frequent maintenance. Mesotherapy can look more affordable per visit, but the more frequent schedule can close that gap over a year. Exosomes sit at a premium price point because of the advanced technology behind them, so they tend to suit patients who are drawn to being at the forefront of regenerative treatment and are comfortable with that investment.

Is one of them “safer”?

All three are considered low-risk procedures, and side effects across the board tend to be mild and short-lived: some redness, swelling, and tenderness at the injection site, which usually settles within a day or two.

That said, there are some sensible distinctions worth knowing:

  • PRP has a particularly low reaction risk because it’s made entirely from your own blood. There’s nothing foreign being introduced.
  • Mesotherapy’s safety profile depends a bit more on the specific formula being used, since it can include a range of active ingredients. This is why having an experienced clinician who knows exactly what they’re injecting matters.
  • Exosomes carry a similarly mild short-term side effect profile, though because the field is newer, the long-term safety data simply hasn’t had as much time to accumulate as PRP’s has. We work only with reputable, quality-controlled exosome products for exactly this reason.

None of these treatments is “zero risk” and the technique and product quality of your provider matters more than which of the three you choose.

So which one is actually right for you?

Here’s how we think about it with our own patients:

Choose PRP if: you want the most well-established, evidence-backed option, you’re dealing with early-to-moderate pattern hair loss, and you’d like a treatment plan with a predictable, well-documented track record behind it.

Choose mesotherapy if: you and your doctor want to build a more customized scalp protocol (maybe even alongside PRP) targeting specific nutritional or scalp-health gaps in addition to follicle stimulation.

Choose exosomes if: you’re drawn to the newest regenerative science, you want to be an early adopter of where hair restoration is heading, and you’re comfortable with a premium investment in exchange for cutting-edge technology.

And here’s the part we want to be really clear about: these aren’t mutually exclusive. Many of our patients combine PRP as their evidence-backed foundation with mesotherapy or exosomes layered in for added support. There’s no rule that says you have to pick just one. The right plan is the one that’s built around your scalp, your goals, and your comfort with where the science currently stands.

“My job is to understand what’s actually happening with your hair and build a plan that makes sense for you specifically. Sometimes that’s one treatment. Often, it’s a thoughtful combination.” — Dr. Ludmila Yaghorova

Frequently asked questions

Is exosome therapy as good as PRP?

It’s better to think of them as complementary rather than competing. PRP has more years of clinical research behind it, which makes it a very dependable starting point. Exosomes are newer and the research is still growing, but the early signals are genuinely encouraging, and we’ve seen good outcomes with our own patients. Many people choose to use both as part of a broader plan rather than picking one over the other.

Can I combine PRP, mesotherapy, and exosomes?

Yes, and it’s quite common. Each works through a slightly different mechanism, so combining them can offer a more complete approach to scalp health. Your doctor can help design a sequence and schedule that makes sense for your specific situation.

Which option has the least downtime?

All three are low-downtime, in-clinic treatments. You can typically return to normal activities the same day, with mild scalp sensitivity for 24–48 hours being the most common after-effect across all three.

Is exosome therapy safe even though it’s newer?

Yes. When sourced from a reputable, quality-controlled supplier and administered by an experienced provider. Short-term side effects reported are mild, similar to PRP and mesotherapy. What’s still developing across the field generally is long-term data, simply because exosome therapy hasn’t been studied for as many years as PRP. We only work with trusted, well-documented exosome products for this reason.

How do I know which one is right for my hair loss?

The honest answer is: it depends on your hair loss pattern, how active your follicles still are, your budget for ongoing maintenance, and your personal comfort with newer versus more established treatments. This is exactly what a consultation is for. We’ll assess your scalp and walk you through which option, or combination, makes the most sense for you.
